Student Physicals and Immunizations records
If you are planning on sending your student back to in-house school this fall, the State of Illinois guidelines for student physicals and immunizations are still in effect.
All information needs to be turned into the school no later that October 15th. Please refer to this document for grade level requirements. Please take advantage of this time to get this done if needed.
